AppointmentsPatients will be seen by appointment only unless there is an emergency. A complete eye exam will last about one hour and includes dilation of the pupil with eye drops(these drops work in approximately 20 minutes). These drops blur your vision and make you sensitive to bright lights as your dilated pupil lets more light into the eye; this effect wears off in about four hours. Dilation may make driving difficult. Please bring your current glasses with you to your eye exam. It is also helpful for you to bring a list of your current medications and any information regarding your medical problems or previous surgery. Children are welcome. Both Dr.’s John and Patrick Costello spent a significant portion of their ophthalmology training treating medical and surgical eye diseases in children. Please call any time during business hours for an appointment. Try to arrive on time and please call if you must break an appointment. (315) 363-1110 (Oneida) PaymentPayment is expected at the time of service; this enables us to keep your costs down. MasterCard, VISA and personal checks are accepted for your convenience. Please remember the responsibility for paying fees resides with you and not your insurance company. Always feel free to discuss fees or billing questions with my staff. Private InsuranceWe do accept assignment for certain private insurances. Please be aware your insurance carrier may require you to make a co-payment for a portion of the bill. Unfortunately, it is not always possible to negotiate a reasonable fee schedule with every insurance company thus assignment is not accepted on all insurances, therefore we will courtesy bill your insurance company. If you require surgery, a courtesy bill will go to your private insurance company for the surgery fees regardless of assignment status. Care CreditA payment plan can be established for procedures such as LASIK, eyelid surgery, or for multifocal implanted lenses with cataract surgery.
